The Sally Sitta CEVA Award recognizes the Commitment to Excellence in Volunteer Administration. The award was established by FADVS in 1996 to recognize outstanding volunteer leaders in healthcare. The award was renamed in 2001 in honor of Sally Sitta, the first recipient of the award. The award is presented annually to an FADVS member that exemplifies our profession.

It's time to recognize your peers by nominating your choice for the 2010 Sally Sitta Commitment to Excellence Award!
It is a user-friendly, easy process:
1. Think of someone who you greatly respect for his/her contribution to the field of volunteer administration.
2. Contact this worthy person and ask for a copy of their current resume or bio via email.
(They will be thrilled to be nominated!)
3. Edit, cut, and paste the information into the easy-to-use CEVA Nomination form provided as an attachment to
this email. Please be sure to read the App Cover Letter carefully. There have been some changes from previous
years. And, please be sure to fill out all the information blanks. Omitted information will delay the nomination
process.
4. Sign it and email it to Joanne Webster, CEVA Chair, and the entire CEVA Committee, by the May 31st deadline.
